Creating meaningful moments…
LEGACY PROJECTS
Our Advantage Health Systems Hospice team strive to implore participation of our patients and/or family members in creating legacy projects.
A legacy project is an exercise that can take many forms, such as booklets, musical playlists, slideshows, collages, and/or poems and songs to recorded interviews. It is our hope to create these projects with the support of our care team of social workers, spiritual counselors and music therapists.
This is a poem one of our therapists helped a patient diagnosed with Alzheimer create for his wife…
